Negative Patient Balances

When reviewing Lytec patient accounts, I often come across out-of-balance situations.  Sometimes they are legitimate, but more often than not things such as negative billing balances are a result of errors in your data entry.  To learn about a few common errors and how to correct them, login and read this.

ANSI 5010 Update

The ANSI 5010 standards for electronic healthcare claims went into effect on January I, 2012. Payers – including Medicare, TRICARE and some state Medicaid plans – are currently rejecting claims that are not in the 5010 standard format.  Rejections typically occur because of the fields or data elements listed below.

Common 5010 Rejections

  • 9-digit ZIP codes for Billing Provider and Facility:  If the last four digits are not populated your clearinghouse likely will put in four default numbers (such as 0000).  However, we are hearing that many payers are rejecting claims with a default Plus-4 ZIP code. The best practice is to capture the 9-digit ZIP code in Lytec so that the additional digits are populated correctly in your 5010 claim.  The 9-digit ZIP code can be easily looked up on the US Postal Service web site.
  • Special Characters: Do not use characters such as #,  :  ‘  - .  The most common character is the # sign. Instead of using this for suite numbers or apartment numbers, spell out “STE” or “APT”
  • NDC numbers: With 5010 NDC numbers are required on ALL injections.  NDC numbers must be 11 characters long.
  • Rendering & Billing Provider: Where the rendering and billing provider is the same, the rendering NPI is not required.

If you have a support agreement with us or have purchased Lytec 2011, we have most likely taken care of these issues during the installation of your software.  Others may need assistance with these issues.

Lytec Training: Appt Schedule

Next Tuesday (1/31 at 11:30AM EST) I am going to review some appointment scheduling features of Lytec 2011.  If you are new to the Lytec appointment schedule, here is a chance to learn the basics.  I will plan to spend about 30 to 45 minutes reviewing the following items:

  • entering a patient appointment
  • tracking appointment status
  • using reason codes
  • viewing past and future appointments
  • the billing column
  • applying a copay
  • printing the schedule
  • printing superbills
  • overview of appt customization

Lytec Default Patient

The Lytec program contains a feature called “default patient”.  The purpose of this feature is to input standard or repetitive information into each new patient chart that you create.  The advantage of this is to prevent the user from forgetting to input information that is critical to processing a clean claim.  Learn more
